My name is Yesenia Castillo, MSN, APRN, FNP-C, and I'm a board-certified Family Nurse Practitioner and the owner of ModernCare Home & Family Medicine, PLLC. I founded ModernCare with a mission to bring compassionate, accessible, and high-quality healthcare to individuals and families across the Rio Grande Valley.
I provide both telemedicine and home visits, allowing my patients to receive the care they need in the most convenient and comfortable setting. My areas of focus include hypertension, diabetes, women's health, wound care, and preventive medicine. I'm passionate about helping my patients take control of their health through education, early intervention, and continuity of care.
