Unique Pediatric Hearts is a Georgia Pediatric Program (GAPP) provider dedicated to helping medically fragile children receive quality care in the comfort of their homes. We provide skilled nursing and personal care services tailored to each childs unique needs. Through the Family Caregiver Option, eligible parents and family caregivers may be compensated for providing approved care to their child. Our compassionate team works closely with families, physicians, and case managers to ensure exceptional care, support, and advocacy. Proudly serving families throughout Georgia, we are committed to improving the quality of life for children with complex medical needs while supporting the families who care for them every day.
