Our home care was established in 1991, and our hospice established in 2005. The health and safety of your patients, caregivers, and family members is our number one priority.

At Triniti Home Health & Hospice we are committed to giving our hospice patients the most comfortable and honoring end-of-life journey possible.

We recognize the unique and difficult challenges of terminal illness, and our goal is to provide emotional, physical, and spiritual support to individuals and their families.

Each client is assigned a nurse case manager who oversees the plan of care from start to finish, increasing visits as needed depending on the client’s condition.

At Triniti, we visit our clients twice as often as the regulated guidelines, as well as implement our own music therapist and massage therapist to aid in the treatment we give our clients.

Our Triniti team provides care for many senior patients, and we feel blessed to hear their stories and learn about their lives. Today and every day, let your senior loved ones know how much they mean to you!

Though grief and grieving look different for everyone, we all experience it throughout our lives. If you or someone you know is dealing with loss of any kind, remember to reach out for support or be a catalyst for helping others heal.

Our volunteers at Triniti are an invaluable part of the care our patients receive. We provide training as needed and will pair your interests with an opportunity that will greatly benefit everyone involved.

One of the many ways in which Triniti helps patients with chronic conditions or post-surgery recovery is with physical therapy. Through physical therapy, patients feel better with less pain, increased relaxation, and reduced need for medications.

At Triniti, we appreciate the staff and volunteers who provide such assistance and feel blessed to be called upon in times of need.
