Trail Creek Place provides residents with a home-like environment, support for activities of daily living, and a community where they can thrive. We develop personalized care plans to meet the needs of each of our residents. Our care team is available 24 hours a day to provide care in our communities. Our residents enjoy a variety of engaging activities, including group games, exercise classes, movie nights, arts and crafts, and social gatherings. We enjoy outdoor activities, such as a walking club, a gardening club, and outdoor events! Trail Creek Place is also a Medicaid Waiver approved community.
