The Lee Health Heart and Vascular Institute Heart Care Transition team works with your cardiologist to ensure you get the follow-up care you need.
It is important for patients with a diagnosis of heart failure, cardiomyopathy or A-fib to go to a follow-up medical appointment shortly after discharge from the hospital.
The Heart Care Transition Clinic has two locations in Fort Myers and one in Cape Coral. In Fort Myers, you can find us inside the main atrium at HealthPark Medical Center and also conveniently located on Metro Parkway near Gulf Coast Medical Center. Our Cape Coral location is situated near Cape Coral Hospital on SE 8th Terrace. At these clinics, you'll be cared for by a specially trained cardiac nurse practitioner who conducts thorough and personalized visits to address your heart health needs.
