I've always had a heart for the elderly. When we visited my great-uncle in his nursing home, I thought, "These people shouldn't be put on the back burner." No matter your age, your health matters. I care for patients post-hospitalization, for Medicare visits, sick visits and annual checkups. Our care model is structured to allow time to build relationships and for patients to fully express all their concerns.
