ElderONE, an affiliate of Rochester Regional Health, is a Program of All-Inclusive Care for the Elder (PACE). Servicing Monroe, Ontario, and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement.
As the area's only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.

ElderONE takes a collaborative approach to caring for seniors. Our goal is to help our participants remain in their home for as long as possible.
