For almost 50 years, Community Health Services (CHS), has been recognized as one of the most respected health systems in the region.

CHS offers a full spectrum of health care services provided by physicians, advanced practitioners, dentists, counselors, and other health care professionals who are dedicated to meeting the highest standards of care.

Community Health Services has eight health centers in northern Ohio. Locations include: Fremont, Fostoria, Stony Ridge, Napoleon, Lima and Willard (a seasonal office).

Everyone is welcome at CHS and we accept all payment types including Medicaid, Medicare, commercial insurance and self-pay patients. We offer a sliding fee schedule based on family size and income.
