The Cardiovascular Health Promotion Disease Prevention Program (CHPDP), located in the Ambulatory Care Pavilion in Valhalla, NY, promotes cardiovascular health through diet, lifestyle changes, medical management, and other interventions. Patients meet with a member of our expert team of providers who will assess overall condition, lifestyle, and health history to determine individual cardiac risk. Then, our team creates a personalized plan to help individuals minimize the risk of developing a heart condition. CHPDP is part of WMCHealth with access to a full array of subspecialty care.
