About Brunswick Orthodontics We can’t wait to welcome you to our office! Once you make your first appointment, the intake forms will be digitally sent for you to fill out at your convenience prior to coming to the office for the initial appointment. Upon your arrival at Brunswick Orthodontics, please notify our receptionist. Dr. Zack will conduct a thorough orthodontic examination, including Diagnostic Records, and explain all aspects of treatment one on one with you.
